Fear of Cancer Recurrence as a Silent Struggle

Breast cancer is the most diagnosed cancer in the world. Despite advancements that have improved survival rates, fear of recurrence remains a common and often overwhelming struggle for survivors. For some, it surfaces sporadically. For others, it becomes a constant undercurrent, influencing every facet of life.

A recent study of breast cancer survivors found that this fear can disrupt emotional, mental, social and professional well-being. Women with more severe fear experienced greater and more frequent disruptions across these areas. Shelley Johns, PsyD, a senior author and researcher-clinician with the Regenstrief Institute, the Indiana University School of Medicine and the IU Melvin and Bren Simon Comprehensive Cancer Center, stated, "Our findings provide clarity about how breast cancer survivors are impacted by fear of recurrence and insight into how they cope with this understandable fear."

Study participants, who were no longer undergoing treatment, were focused on rebuilding their lives. Despite being disease free, many found that fear of recurrence had a significant hold on their daily experience. Women with mild fear reported occasional disruptions, such as sleepless nights before medical check-ups. Those with more severe fear described frequent and consuming anxiety, often withdrawing from social events or isolating themselves to block out intrusive thoughts. Approximately 74 percent of participants reported experiencing clinical levels of fear.

The survey, completed by 347 women, revealed a range of emotional and psychological impacts. Many described recurring stress, sadness and irritability. Some struggled to focus at work as thoughts about their cancer interrupted their concentration. Others expressed embarrassment, feeling they were more fearful than they should be compared to other survivors. A few found it painful to be around family, constantly questioning how much time they had left with their loved ones.

The findings offer a detailed look at how fear of recurrence affects breast cancer survivors and provide valuable insights into the coping strategies that may help reduce its hold.
